The Role of Health Locus of Control in Predicting Adherence to Treatment and Illness Perception, with the mediating role of Health Literacy and Differentiation of Self in women with Polycystic Ovary Syndrome (PCOS)

Introduction: The health locus of control, health literacy and self-differentiation are among the factors influencing adherence to treatment and illness perception. The present study aimed to predict the role of health locus of control in forecasting adherence to treatment and illness perception, considering the mediating roles of health literacy and self-differentiation in women with polycystic ovary syndrome (PCOS).
Method: The research method was descriptive correlational. The statistical population consisted of women with polycystic ovary syndrome (PCOS) who visited clinics, outpatient centers, and private practices in districts 1 and 3 of Tehran during the first quarter of 2024. Among them, 233 participants were selected through purposive sampling. The research instruments included a Demographic Questionnaire, the “Multidimensional Health Locus of Control Scale”, the “Self-Report Adherence to Treatment Questionnaire”, the “Brief Illness Perception Questionnaire”, the “Evaluation of Health Literacy for Iranian Adults”, and the “Differentiation of Self Inventory”. The validity of the instruments was assessed qualitatively through content validity, and reliability was measured by internal consistency using Cronbach’s alpha coefficient. Data analysis was performed using SPSS .24 and AMOS. 24 software.
Results: The health locus of control predicted treatment adherence with the mediating role of health literacy in women with polycystic ovary syndrome (β= 0.446, P=0.010). The health locus of control predicted treatment adherence with the mediating role of self-differentiation in women with polycystic ovary syndrome (β=0.169, P=0.010). The health locus of control predicted treatment perception with the mediating role of health literacy in women with polycystic ovary syndrome (β=0.404, P=0.010). The health locus of control predicted treatment perception with the mediating role of self-differentiation in women with polycystic ovary syndrome (β=0.188, P=0.010)
Conclusion: The treatment adherence and disease perception are predicted based on the health locus of control with the mediating roles of health literacy and self-differentiation in women with polycystic ovary syndrome. Therefore, it is recommended that health counselors and psychologists enhance the health locus of control in women with polycystic ovary syndrome to promote treatment adherence and better disease perception in this population. Strengthening self-differentiation can be an important therapeutic goal in psychotherapy.
